Parental Consent Form (Atto di Assenso)

In the case of a minor travelling to Italy accompanied by only one of the parents and/or a grandparent/coach/mentor, the written consent of both parents is required permitting the same.

For this purpose, the parents as well as the minor (and in the given case, the person whom the minor is travelling with) are required to visit the Consulate, with prior appointment, in order to complete the formalities and sign the Consent form in front of the Consular Office.

In order to obtain an appointment, there are two steps to be followed

 

Step 1:

Scans of the following documents are required to be shared by the parents via mail at the email address consolaremumbai@esteri.it:

1)      Front and back page of the passport of the minor

2)      Front and back page of the passport of the mother

3)      Front and back page of the passport of the father

4)      In case of the child travelling with another adult apart from the parents, front and back page of the passport of the adult accompanying the minor.

5)      Birth certificate of the minor

6)      Receipt of the visa appointment booking for the minor.

7)      In case of a discrepancy in the name of the full name of the parents’ mentioned in the child’s birth certificate and passport (last page) and the name mentioned in both the parents’ passports, a One and the Same Name certificate is required to be submitted. One And The Same Certificate is required to be issued by the Tehsildar/the Municipal corporation (Registrar of Births and Deaths) or Panchayat. The document  is required to be attested by the Home Department of the State and then Apostilled by the MEA .
Affidavits and Self-Declarations will not be accepted.

Please note that any discrepancy in the name across documents will not allow this Office to proceed further with releasing the consent form unless and until a One and the Same Name certificate is submitted.

Once the documents are verified by us, an email will be sent requesting the parents to proceed with the second step.

 

Step 2:

An appointment to visit the Consular office to sign the consent form will have to be booked via the portal  Home Page – Prenot@Mi (esteri.it)

Once the appointment is confirmed, an email will be sent via this email address.

The appointment will be considered confirmed ONLY after receiving an email also from this email address.
A confirmation status on the portal does not guarantee a confirmed appointment.

On the day of the appointment, the parents as well as the minor will have to be present at the Consulate and will have to present the originals as well as the photocopies of the documents mentioned in the previous step.

The cost of the attestation is of Rs. 2070/- ( till the 31st March 2026 ) and is required to be paid exclusively via UPI.
The payment is required to be executed directly at this office, on the day of the appointment.
