A request for transcription of a civil status act can be submitted to the Consular Registry by sending the originals of the documents by regular mail or private courier to the following address:

Before sending the originals, it is advisable to forward copies of all documentation to the e-mail address consolare.mumbai@esteri.it for appropriate verification and a response will be provided to you.
Procedure for the transcription in Italy of civil status records (Birth; Marriage; Death)
The civil status document must be delivered or sent to the Consulate for subsequent transmission to the competent Italian Municipality in original with the following attestations.
For documents issued in the Jurisdiction of Mumbai except for the state of Goa
- Certification by the competent Home Department
- Apostille issued by the Ministry of Foreign Affairs of the Indian Government
- Italian translation to be attested for conformity to the original text by the Consulate
For documents issued in the State of Goa
- Certification by the Collector
- Attestation of the competent Home Department
- Apostille issued by the Ministry of Foreign Affairs of the Indian Government
- Translation into Italian to be certified as conforming to the original text by the Consulate
TRANSCRIPTION
1) Obtain the birth certificate registered at the competent Office (Registrar of Birth and Death) in original – taking care to verify the correctness of the names entered – in duplicate: one will be retained by this Office.
2) Have the said certificate (ORIGINAL) legalised (at the competent Home Department) and then Apostilled at the Ministry of Foreign Affairs in New Delhi, or at the branches in Hyderabad and Chennai where, at the Secretariat offices, there is an office that affixes the Apostille, or again at the Passport Offices in Mumbai, Panjim, Thiruvanthapuram >> http://mea.gov.in/apostille.htm).
3)Translate or have the text translated into Italian. Please note that this office does not carry out translations. Attached is a list of translators known to us and working in the area. It is also possible to translate the entire text yourself.
4) Finally, submit to this Consular Office the above-mentioned document with its translation, together with copies of the valid passports of the parents and the attached transcription request form. The documents must be received in original.
NB: Marriages of any religious belief cannot be transcribed.
